Forex Trading Strategies And Systems

I've been trading the forex markets for several years now so I've developed quite a few different systems in my time. However there are some that are more profitable than others, so let me share you with you some of my most profitable forex trading strategies.
4 Hour Trading Strategy
I created this trading strategy myself and have been using it for several years now. This one system has generated more profits that any other system I have ever used, and yet it's surprisingly simple.
I simply look at the daily trend for a particular currency pair (usually the GBP/USD, EUR/USD or USD/JPY pair) using a very simple but effective technical indicator, then I wait for two EMAs (exponential moving averages) to cross over in the same direction on the 4 hour chart.
I will then enter a position (usually after a slight pull-back) and will employ a two-part exit strategy to maximise my profits. One half of the position will be closed out early for a safe profit, and the other half will be left to run for as long as possible in order to capture those really big price moves.
As I say, this particular forex trading strategy is highly effective, as regular readers of my blog will know because I share my trading results every week in my 'Weekly Trading Updates'.

The only problem with trading this strategy is that there will always be quiet periods and particular days where you know you are not going to get any set-ups on any of the major currency pairs. Therefore at times like these I will often employ some of the other trading methods that I keep in reserve:
CCI Divergence Trading System
This is a forex system that I've recently created and it basically uses the popular CCI indicator with two different settings. The key here is to wait until there is divergence between both of the CCI indicators at the same time because this will give you a set-up with a very high success rate.
You don't get that many good set-ups per day using this trading strategy, but when you do, you are likely to make some decent profits because it is a very high probability set-up.

Forex Income Engine 2.0 Methods
At the time of writing (20 June 2009) I've just started using the three day trading methods included in the Forex Income Engine 2.0 course as well. I've always been quite sceptical about many of the short-term forex methods that I come across, but I've been very impressed with these three methods so far because they do actually produce some very good returns.

Forex Nitty Gritty Method
This trading method was included in the Forex Nitty Gritty course and although it is a very basic method, it is actually surprisingly effective. The goal is to look for pairs that are in strong upward or downward trends, wait for a pull-back, and then enter a trade if the trend continues.
I've been using this method on the 15 minute charts for quite a while now and it has always performed well for me because these continuation trends occur all the time.

Long-Term Trading Strategy
I'm not really a long-term trader but I do occasionally open a position if a good trading opportunity arises. I will usually use the daily charts for these trades and will look at a variety of indicators such as the 200 day moving average, the supertrend indicator, established support and resistance levels, fibonacci levels if applicable, and Marketclub's excellent trading signals.
I will regularly post my long-term analysis of the various currency pairs on this blog, but I will only follow this up with an actual trade if I'm really confident about my predictions.
Other Forex Trading Strategies
Finally as well as all of the trading systems and strategies listed on this page, I also have a few breakout strategies that I like to use when a good opportunity presents itself. I'm also constantly testing out new ideas and reviewing the various trading systems that I get sent regularly by product owners who want me to promote their product.
However for the most part it's my 4 hour trading strategy that I spend most time on because this is my core system which generates the most consistent and reliable profits. All of the other forex trading systems are used to boost my trading pot during the quieter periods of the week.

Weekly Trading Update - 03-07 October 2011

It's been a very quiet week this week because I only managed to find one decent trading opportunity overall. There were no trades according to my 4 hour trading system (see right for more details) because the price generally moved against the trend on the FTSE and the GBP/USD and EUR/USD pairs. The only trade I did find was a breakout trade on Tuesday morning.
The overnight trading range (between 00.00 and 08.00 UK time) for the GBP/USD pair was just 44 points so when the price finally broke below this range at around 08.55, I went short at 1.5401.
I then closed the position for 20 points profit a short while after. The S1 level, which is often a good exit point was a little too far away for my liking (around 50 points below my entry point), but as it turned out the price did actually hit this target.
Not to worry though. A 20 point profit for the week is better than no profit at all, and certainly better than losing money. Plus the stock market rally has meant that a lot of my long term holdings have gone up nicely, and I even banked a 150 point profit going long on Vedanta earlier in the week.
So it wasn't a bad week at all. Looking ahead, there could be some decent trading opportunities next week if we get the slightest whiff of bad news because the indices and the GBP/USD and EUR/USD are all geared up for another downward EMA crossover on the 4 hour chart. They are all still in downward trends on the daily chart according to the Supertrend indicator, so this could well happen.

Weekly Trading Update - 10-14 October 2011

There was just the one trade this week, and that was based on my early morning breakout system. My 4 hour trading system (see right for more details) didn't throw up any opportunities, but we are in a transitional phase at the moment with the daily Supertrend indicator turning positive (or about to turn positive) on the FTSE and some of the major currency pairs.
Anyway with regards to my one breakout trade, it was on the GBP/USD pair as always. The overnight range on the GBP/USD pair was generally too wide on most days, but yesterday it was just 38 points. So I waited for the price to break out of this range after 8.00 (UK time), and it did so (breaking downwards) just after 9.00.
I then went short at 1.5723, with my initial price target being the pivot point. This was only around 11 points lower, so I closed half the position at this level, moved my stop loss down to break-even and targeted 1.5700 for the second half of the trade. Thankfully this second target was hit and it was a reasonable profit overall.
So although the forex profits were quite small this week, I cannot really complain because the stock markets have continued to strengthen, and therefore the value of my long term share portfolio has risen nicely.

Weekly Trading Update - 17-21 October 2011

I didn't place any forex trades at all this week and it was simply because I was too distracted by the share price movements of Tomco Energy, which is now up over 50% this week (and was up 100% at one stage).
I mainly invest in FTSE 100 stocks these days, but this is the one AIM stock that I hold in my share portfolio and it's finally burst into life. I have several million shares in this oil shale company and a few spread betting positions as well.
I'm holding out for at least 10p in the long run, and hopefully a lot more, but in the meantime I have been banking some massive profits from two of my spread betting positions (and I have since opened one more long position), which is why I couldn't be bothered to trade forex this week.
In fact I haven't even looked at a forex chart at all this week until a few minutes ago. Looking at the charts I don't think I missed much. It's looks as if there was one breakout opportunity that would have yielded around 20 points, and a few false EMA crossovers on the 4 hour charts which would definitely have resulted in losses.
So it's probably best that I didn't trade any positions this week as most of the major pairs, and indeed the major stock market indices, seem to be trading sideways and lacking any real direction at the moment. Hopefully we will start to see some definite trends emerging in the next few weeks.
(Incidentally for legal reasons I should point out that in no way am I recommending that you buy shares in Tomco Energy. This is a high risk stock and the only reason I mentioned it was to explain why I didn't open any forex positions this week).

Forex Books On The Kindle

Today I want to talk about some of the forex books you can buy on the Kindle. If you haven't yet bought a Kindle (which you can buy here from Amazon.com or Amazon.co.uk), I highly recommend you do so because I bought one earlier this year and it's one of the best things I've ever bought.

It is essentially an ebook reader that stores thousands of books, and allows you to read any of the books you have in your collection (and download new books) on the move whenever you like.
I've mainly being using it to read travel books (to compensate for the fact that I have been unable to go anywhere since last year because I haven't been well enough), but I also like to read lots of business and finance books, particularly those that cover stock market investing and currency trading.

The great thing about having a Kindle is that you can choose from thousands of ebooks on Amazon, and they are usually slightly cheaper than the actual books themselves. Plus with regards to forex trading ebooks, they are now capable of displaying the exact same charts and tables as the printed books, which wasn't always the case when the Kindle first came out.

As I say there is a great range of books to choose from once you own a Kindle. You still have slightly more choice if you buy a printed book because not every single book is available in ebook form. However you will still find plenty of books to choose from.

Some of the ebooks are even free from time to time, and if you are patient enough you will often find that the price of a particular ebook will drop significantly at some point.

You don't need to spend a fortune to learn about a particular subject either. For example with regards to forex trading, here's a selection of currency trading ebooks that you can buy and download to your Kindle for less than $10 on Amazon.com:

Weekly Trading Update - 24-28 October 2011

Well it's been an excellent week for me this week. However that's nothing to do with my forex trading. It's mainly because the stock market has rallied and my stock portfolio is now in much better shape. I have banked some nice profits in the likes of BP and Rio Tinto, but I'm still holding several stocks for the long term, including Tomco Energy, which is still doing very nicely.
On the forex front, it has been more frustrating. There were excellent opportunities to go long on the GBP/USD and EUR/USD pairs using my main 4 hour trading system (see right for more details). However both the upward EMA crossovers took place in the overnight trading session (when the Eurozone bailout plan was announced), so I was unable to trade them.
My breakout system didn't help generate any profits either. There was a breakout today that I missed because I was getting my breakfast (although it has moved no more than 10 points into profit so far), and the one trade I did place resulted in a 20 point loss.
I'm starting to think it's time to reassess my trading strategies and maybe go through my vast collection of trading strategies to try and find something different. I don't seem to be making anywhere near as much money as I used to, and I'm not really enjoying trading the forex markets as a result.
In fact I much prefer trading and investing in stocks, and always have done. So maybe that's where my future lies.

England is Not an Independent Country

1. Has space or territory that has internationally recognized boundaries (boundary disputes are OK).
Yes, England does have internationally recognized boundaries and is 130,395 square kilometers in area. England is the largest component of the United Kingdom of Great Britain and Northern Ireland.
2. Has people who live there on an ongoing basis.
Yes, according to the 2001 census, England's population is 49,138,831.
3. Has economic activity and an organized economy. A country regulates foreign and domestic trade and issues money.
Somewhat. England certainly has economic activity and an organized economy. However, England does not regulate its own foreign or domestic trade, the United Kingdom's parliament acts on the entire nation's behalf.
The Bank of England serves as the central bank for the United Kingdom and does print banknotes for both England and Wales.
4. Has the power of social engineering, such as education.
No. National government departments (such as the Department for Education and Skill) at the national level maintain responsibility for social engineering.
5. Has a transportation system for moving goods and people.
Mostly. England has a transportation system but the system is not fully under English control. There is no English Parliament but some systems are under local control but must of the national transportation system is under national control by Parliament.
6. Has a government that provides public services and police power.
Somewhat. Local governments provides local law enforcement and fire protection but the national government controls criminal and civil law, the prosecution system, the courts, defense and national security across the United Kingdom. England does not and can not have its own army.

Muammar Gaddafi: How he died

ol Muammar Gaddafi died from bullet wounds some time after a failed attempt to escape from the fighters of the National Transitional Council (NTC), but the exact circumstances of his death are still emerging.

After the fall of Tripoli in August, Sirte remained one of the final pockets of loyalist resistance, in particular District 2 in the north-west of the city.
In the early hours of Thursday it appears that Col Gaddafi, accompanied by key loyalists, decided to attempt a breakout from District 2 in a convoy of vehicles.
At about 08:30 local time French aircraft operating as part of the Nato mission attacked the convoy of 75 vehicles heading out of Sirte at high speed approximately 3-4 km (two miles) west of the city near the western roundabout.
Among those in the convoy were Col Gaddafi's son Mutassim and head of Gaddafi's army Abu Bakr Younis Jabr - both men were later reported dead at the scene and Mutassim's body shown on Libyan TV.
According to Nato, a first strike destroyed one vehicle and caused the convoy to disperse into several groups.
One of those groups, carrying Col Gaddafi, headed south and was hit again by a Nato fighter, destroying 11 vehicles.

Col Gaddafi and a handful of his men managed to escape on foot and sought refuge in two large drainage pipes filled with rubbish. Rebel forces then closed in.
Fighter Salem Bakeer told Reuters: "At first we fired at them with anti-aircraft guns, but it was no use.
"Then we went in on foot. One of Gaddafi's men came out waving his rifle in the air... as soon as he saw my face he started shooting at me. I think Gaddafi must have told them to stop. 'My master is here, my master is here', he said, 'Muammar Gaddafi is here and he is wounded'".

What would Chinese cash for eurozone bailout fund mean?

China has plenty of money and would be able to help, though it is not a given that a cash injection will be beneficial for European nations
With some $3.2 trillion (£2tn) in its foreign reserves coffers, China may well be a natural contributor to the eurozone's bailout fund.
But the fact that China is a wealthy country, and therefore able to help, does not explain why China would actually want to do so.
One obvious reason might be a desire to prevent the crisis from leading to a recession in Europe - China's biggest export market.
Moreover, if the eurozone crisis were to spread globally - just as the US mortgage crisis did in 2008 - not only could it damage China's other trade partners, but also China itself.
Beijing recently had to intervene to shore up its own banks, which many investors fear are riddled with bad debts. Another global financial crisis could leave those banks in even worse shape.
Feeling the pinch With consumers in Europe - and indeed in the US - already scaling back consumption and repaying personal debts, the demand for stuff made in China is slowing and could stay weak for years.

On the domestic front in indebted European countries, a reduction in imports is generally deemed beneficial, as it helps reduce their trade deficits. Or to put it another way, if consumers cut back, it is better for a country if they spend less on stuff made abroad and maintain spending on goods made at home.
But of course, China is not the only one that will take a hit as global consumption and trade slows to a trickle. Exporters - especially in trade surplus countries such as China, Japan or Germany, but also in trade deficit countries - will feel the pinch.
In turn, this general reduction in global trade will cement stubbornly high unemployment levels in the industrialised world, doing little to alleviate the crisis or to reduce the risks of it spreading beyond Europe.
But to the extent that the burden of shrinking demand falls on exporters like China, that means more of the job losses will be Chinese jobs.
Indeed, as global demand weakens, there is less point in keeping up massive investment in manufacturing in China. So perhaps it would be better to invest elsewhere?
Sound investments China has long said it is eager to invest in Europe. But there are different ways for it to do so.

The one discussed in the context of the eurozone bailout fund would be an investment in European bonds, which equates to lending money to European governments.
This is not to say China is about to lend money directly to countries that need it the most.
Buying Spanish or Italian bonds may not be a tempting proposition, while buying German bonds clearly is, as they are deemed more likely to be repaid in full.
China has made it clear that it would only want to make sound investments, so it would require guarantees.
A Chinese contribution to the European European Financial Stability Facility (EFSF) would thus, in practice, differ little from a loan to Germany.
And that, of itself, would do little to help the likes of Spain and Italy manage their finances.
Indeed, Michael Pettis, international finance professor at Beijing University, says that Europe - or at least Germany - has plenty of capital of its own, and shouldn't even need China's money.

He says that if China invests more capital in Europe, it may perversely end up putting more Europeans out of work.
By increasing its total investment in the eurozone, China is likely to push up the euro's value.
And that would make the eurozone's exports less competitive in international markets and Chinese exports more competitive in Europe - hardly an outcome that will help struggling Mediterranean countries.
Direct investment Moreover, investing in government bonds is a very different proposition from investing in assets such as buildings, factories or infrastructure - the sort of investments that would deliver economic growth and create jobs.
Such investment is hard to come by these days, and transferring eurozone government debts from Europe's banks to China is not expected to do much to alleviate the situation.
As European governments repay their loans to the banks, the cash is likely to be absorbed by banks to help them with their recapitalisation. So it may not result in fresh funds being freed up for loans to companies.
In other words, Chinese investment in European bonds may do little to bolster economic growth in Europe.
Chinese investment directly into the real economy in Europe could well have a much greater effect.
Whether European voters would welcome Chinese buyers of real estate, companies and roads at a time when prices are depressed by the ongoing crisis is another matter.
But compared with the non-tangible rewards China is likely to demand in return for formal financial support - such as an early European Union recognition as a market economy, greater voting rights within the International Monetary Fund, a lifting of a ban on European arms sales to China, or silence around the matter of China's supposed efforts to keep its currency artificially weak - that may be a small price to pay.

International prosecutors have had "informal contact" with the son of slain ex-Libyan leader Muammar Gaddafi.
The International Criminal Court (ICC) said intermediaries had been used in indirect talks with Saif al-Islam.
Prosecutors said the court had made it clear to Gaddafi's son, who is wanted for crimes against humanity, that he was innocent until proven guilty.
Saif al-Islam, who was once the presumed successor to his father, has been in hiding for months.
Recent reports claimed he was in a convoy heading toward Libya's desert border with Niger, where other Gaddafi allies have fled.
Zimbabwe-bound? ICC prosecutor Luis Moreno Ocampo, whose office issued an arrest warrant for Saif al-Islam in June, said in a statement that the ICC wanted him to face trial.
"The office of the prosecutor has made it clear that if he surrenders to the ICC, he has the right to be heard in court, he is innocent until proven guilty. The judges will decide," the statement said.
Mr Moreno Ocampo said the ICC had learnt "through informal channels" that a group of mercenaries were offering to move Saif al-Islam to a country that has not signed up to the ICC's rules.
"The office of the prosecutor is also exploring the possibility to intercept any plane within the airspace of a state party in order to make an arrest," the statement said.
Reports say Zimbabwe is a likely final destination for Saif al-Islam, if he chooses to flee from the ICC.
Zimbabwean President Robert Mugabe was a long-time ally of Muammar Gaddafi.
The former Libyan leader, who was deposed in August after six months of civil conflict, died from gunshot wounds last week after fierce fighting in the city of Sirte.

Sunderland's Titus Bramble charged with sexual assault

Sunderland footballer Titus Bramble has been charged with two counts of sexual assault and urinating in a public place.
The 30-year-old defender, from Wynyard, Teesside, will appear at Teesside Magistrates Court on 10 November.
The Premier League star was arrested on 28 September and 14 October on suspicion of carrying out separate sexual assaults.
Mr Bramble's lawyer said he would "vigorously contest" the allegations.
A Sunderland AFC spokesperson said: "Titus Bramble is training independently at present and is not involved in any first team duties.
"This will remain the case, pending the outcome of club investigations."
Mr Bramble began his footballing career at Ipswich, before making a £6m move to Newcastle United.
He then joined Wigan Athletic and moved back to the North East with Sunderland last year.

RÉPONSE

Le premier scanner a été réexaminé avec attention et les
2 lésions, évidentes, étaient situées dans la loge amygdalienne
gauche (fig. 3a, b et c). Dans la loge amygdalienne
droite, des microcalcifications étaient également
visibles.
Le diagnostic est celui d’une lithiase amygdalienne
(« tonsillolith » des Anglo-Saxons).
Sous anesthésie générale, après une incision à la face
postérieure du pilier amygdalien antérieur, 2 lithiases
amygdaliennes ont été enlevées sans difficulté (fig. 4). La
même technique a été appliquée du côté controlatéral
pour extraire les microcalcifications.
L’orthopantomogramme postopératoire a confirmé
l’exérèse des 2 lésions.
Le patient a été revu en consultation à 1 mois et à
3 mois. Depuis l’intervention, les douleurs ont disparu et il
n’y a eu aucun épisode fébrile.

Aspects épidémiologiques
L’incidence exacte des carcinomes de la lèvre est difficile à
évaluer car ils font l’objet d’une approche épidémiologique
globale avec les cancers de la cavité buccale, du pharynx et
du larynx (voies aérodigestives supérieures [VADS]) et les
cancers de l’oesophage. Certaines caractéristiques sont en effet
communes, parmi lesquelles le fait qu’ils soient souvent liés au
tabagisme et à la consommation excessive d’alcool. Les derniers
indicateurs et données en France [1] figurent dans le
tableau I.
Les 24 500 nouveaux cas de cancers des VADS et de l’oesophage
observés correspondent pour 63 % à la sphère « lèvres,
cavité buccale, pharynx » (pour 17 % au larynx et pour 20 % à
l’oesophage). Selon les enquêtes, la distribution précise par
sous-localisation varie, elle n’est pas toujours disponible en
France. En 1995, sur un total de 21 597 cancers des VADS
(14 926 chez l’homme et 6 671 chez la femme), 410 localisations
labiales (code 140 dans la classification ICD-9, et C00 dans
l’ICD-10) ont été enregistrées (375 hommes et 35 femmes)
ayant entraîné 104 décès (92 hommes et 12 femmes) [2].
Dans les localisations VADS, le cancer des lèvres est en
7e position chez l’homme et en 9e position chez la femme
par ordre de fréquence. Il représente 6,6 % des cancers
buccaux en France (2 % seulement dans notre série à l’hôpital
de la Salpêtrière) [3].
Ces chiffres sont nettement inférieurs à ceux rapportés de
manière plus générale chez les caucasiens (race blanche), où
le cancer labial représente 25 à 30 % de tous les cancers
buccaux [4,5]. Cette variation est probablement due à une
implication différente des principaux facteurs de risque : radiations
ultraviolettes (UV) et tabac.
Le carcinome épidermoïde labial est un cancer de l’homme
d’âge mûr. Le sex-ratio est de 10 à 20 :1. La femme est affectée
dans 2 % à 2,8 % des cas ; 90% des patients ont plus de 45 ans
et 50 % ont 65 ans et plus [4]. Le carcinome de la lèvre rouge
est beaucoup plus rare chez les ethnies à peau foncée dont les
individus à peau jaune.
Facteurs étiologiques
L’exposition chronique au soleil (UV) constitue un risque
admis de carcinome labial prédominant chez les personnes
à peau claire ayant vécu au grand air, exposées au soleil et
aux intempéries (paysans, marins) ou vivant dans des latitudes
très ensoleillées (Australie, Texas. . .). Le rôle carcinogène
des UV B est démontré et apparaît prépondérant
devant les UV A et C. Le risque augmente avec la durée de
l’exposition et l’âge, avec un effet seuil. C’est souvent sur des
lésions de chéilite actinique que le carcinome épidermoïde
peut se développer. Son incidence n’est toutefois pas toujours
corrélée à l’exposition au soleil. Comme on l’a vu, d’autres
facteurs exogènes interviennent et ont une action synergique,
en particulier le tabagisme. Le tabac qui peut être
consommé de diverses manières (cigarette, pipe, chique
etc. . .) peut être responsable de kératoses ou leucoplasies

Conclusion
Concernant les tumeurs limitées, la chirurgie fait appel aux
techniques de conservation fonctionnelle, tandis que pour les
tumeurs avancées, les pertes de substance sont traitées par
des techniques de reconstruction complexes, notamment par
les lambeaux libres.
La radiothérapie conformationnelle apporte un progrès indiscutable
dans la prise en charge de ces tumeurs tant sur le plan
de l’efficacité que des effets adverses. Pour les tumeurs avancées,
la radiochimiothérapie concomitante et la radiothérapie
hyperfractionnée (escalade de doses) ont montré une augmentation
modérée mais significative des taux de survie et
de contrôle locorégional. La radiochimiothérapie postopératoire
est indiquée dans les formes de mauvais pronostic chez
les sujets âgés de moins de 70 ans. L’association du cisplatine,
du 5-FU et du taxotère est le nouveau standard de la chimiothérapie
d’induction pour la préservation laryngée dans les
tumeurs avancées (T3-T4).
L’association d’une thérapie moléculaire ciblée à la radiothérapie
a montré sa supériorité (par rapport à la radiothérapie
exclusive) dans la survie pour les tumeurs avancées.
Le traitement des récidives locales dépend des possibilités de
rattrapage chirurgical et/ou de la réirradiation dont les indications
restent souvent limitées.
L’efficacité de certaines molécules en phase 2 métastatique
n’a jamais été confirmée dans les phases 3 où le bras de référence
comprenait des sels de platine.
La classification TNM reste le meilleur indicateur pronostique
de la survie.
Si les progrès thérapeutiques ont indiscutablement apporté
une amélioration en termes de morbidité et de qualité de
vie, et si les petites tumeurs gardent un pronostic acceptable,
avec un contrôle local dans 80 % des cas, dans les tumeurs
avancées le contrôle local est obtenu dans seulement 30 à
60 % des cas et la survie à 5 ans est de 15 à 40 %.
Concernant les thérapeutiques classiques de ces tumeurs ORL,
les progrès potentiels que l’on peut attendre, notamment en
balistique de radiothérapie, en chirurgie réparatrice avec les
lambeaux libres, avec de nouvelles drogues cytotoxiques en
chimiothérapie, amélioreront très certainement le pronostic
de ces tumeurs. C’est l’association de ces thérapies standards
qui permettra un meilleur contrôle tumoral. Le développement
des essais cliniques coopératifs est un impératif pour tester
objectivement ces nouvelles stratégies.
C’est certainement en biologie moléculaire que les progrès
sont à attendre en améliorant la connaissance de la signalétique
des cellules tumorales, pour mettre en évidence de nouvelles
cibles thérapeutiques.

Les carcinomes épidermoïdes labiaux relèvent de la chirurgie
d’exérèse avec plastie de reconstruction (techniques de Dieffenbach,
Bernard, Abbé, Estlander, Gillies, McGregor, Ginestet,
Meyer et Shapiro, Johansen, Fries, etc.) [10].
L’aspect fonctionnel est un élément primordial de toute reconstruction
labiale.
Les carcinomes de plus de 2 cm de diamètre associent un
évidement ganglionnaire cervical prophylactique, éventuellement
combiné avec la radiothérapie selon le nombre de ganglions
atteints et la présence de ruptures capsulaires. Pour
certains, un évidement ganglionnaire au moins sus-hyoïdien
est indiqué d’emblée pour toute tumeur supérieure à 1 cm.
La technique d’exérèse par étapes de Mohs nécessite une
équipe entraînée et demeure peu pratiquée en France.
La radiothérapie directe sur la lesion, et particulièrement, la
curiethérapie est actuellement exceptionnelle.
Le taux de survie à 5 ans est > 80 % pour tous les auteurs et
peut atteindre 96,7

Cancers du versant muqueux de la lèvre inférieure
Cette localisation est fréquente dans les populations qui ont
l’habitude de garder du tabac dans le vestibule buccal, surtout
en Inde, au sud est asiatique mais aussi au Soudan [4]; le tabac
est maintenu dans le vestibule, soit avec de la chaux éteinte
(« catachu ») soit avec une noix ou feuille de betel (« pan ») ou
noix d’Arèque. Au Cambodge, le bétel chiqué est une habitude
féminine, et ce type de cancer est le plus prépondérant chez la
femme. Enfin chez les patients qui placent du tabac à priser
dans le vestibule buccal inférieur, des leucoplasies typiques peu
symptomatiques se développent, uniformes mais mal
circonscrites (figure 6). C’est en particulier le cas aux Etats-Unis
surtout chez les femmes (snuff dipper’s keratosis). L’âge
avancé et une consommation anormalement élevée d’alcool
sont des facteurs favorisant l’apparition du cancer.
Le carcinome épidermoïde adénoïde kystique ou cylindrome,
rare dans cette localisation, n’est qu’une variante du carcinome
épidermoïde se développant à partir des glandes salivaires
accessoires de la face interne des lèvres.
Cancers de la commissure labiale
Ces tumeurs sont rares (4 % des cancers des lèvres). La lésion
est une ulcération fissuraire à base indurée dont l’extension se
produit avec prédilection vers la muqueuse jugale et non la
peau ; elle siège dans certains cas uniquement sur la muqueuse
rétrocommissurale, affleurant la commissure proprement dite.
Elle succède généralement à une leucoplasie le plus souvent
d’origine tabagique (en Inde particulièrement, du fait de fumer
le « bidi », cigarette à bon marché). La présence chronique de
Candida albicans est fréquente, faisant discuter la possibilité
que la lésion soit, à l’origine, celle d’une candidose chronique.
Des formes verruqueuses de carcinome peuvent être observées
aux commissures labiales. Il s’agit le plus souvent de transformation
maligne d’un carcinome verruqueux d’Ackerman (ou
papillomateuse orale floride) ou de l’évolution d’une PVL
(Proliferative verrucous leucoplakia) [7]. Dans le premier
cas, il s’agit souvent d’une lésion papillomateuse jugale plus
ou moins verruqueuse qui s’étend en nappe et atteint la
commissure (figure 7). Différents stades histologiques (stade
I, aspect de papillome avec gros bourgeons épithéliaux renflés
à la base), stade II avec apparition de petits bourgeons secondaires
sur les faces latérales des papilles et épaississement des
couches cellulaires basales) rendent compte de l’évolution
inexorable de ces lésions vers un carcinome infiltrant (stade
III). Les types 16 et 11 du HPV ont été détectés dans ces lésions
orales. Le traitement est chirurgical suivi d’une surveillance
rapprochée pour détecter les récidives, fréquentes. L’irradiation
de ces lésions n’est pas recommandée en raison du risque
d’évolution vers des carcinomes anaplasiques. Des atteintes
ganglionnaires ont été occasionnellement rapportées et les
métastases sont rares.
Dans la PVL, la lésion de départ peut être une leucoplasie
homogène évoluant progressivement pour devenir inhomogène
avec des dysplasies, puis éventuellement nodulaire
et/ou verruqueuse avec transformation maligne.

Histopathologie
Le carcinome épidermoïde se développe sur une muqueuse
d’apparence saine ou atteinte d’une précancérose originelle
que l’on peut encore parfois reconnaître [9]. C’est une prolifération
épithéliale maligne développée aux dépens des kératinocytes.
Selon le degré d’infiltration et de franchissement de
la membrane basale, on parle de carcinome in situ (ou intraépithélial
ou dysplasie sévère), de carcinome micro-invasif ou
de carcinome invasif.
Dans le carcinome in situ, il existe une transformation segmentaire
de l’épithélium portant sur toute sa hauteur sans
modifications de la membrane basale. L’épithélium est irrégulièrement
stratifié, avec des noyaux de forme et de taille
inégales, hyperchromatiques et des mitoses visibles jusqu’en
surface. Dans le carcinome micro-invasif l’aspect est proche,
mais on détecte également quelques brèches dans la basale
avec effraction de cellules carcinomateuses dans le chorion. Le

Monsieur G.B., aˆge´ de 42 ans, a e´te´ adresse´ a` la consultation
car il pre´sentait depuis un mois une geˆne pharynge´e de type
angine.
Les seuls ante´ce´dents notables e´taient d’ordre dermatologique
et assez re´cents : pitiriasis rose´ de Gilbert associe´ a` un
prurigo, deux e´pisodes de prurit ge´ne´ralise´ conside´re´s
comme une gale et traite´s par ivermectine (StromectolW) a`
neuf reprises en l’espace de deux mois.
L’examen clinique endobuccal retrouvait des le´sions multiples
et he´te´roge`nes avec notamment des le´sions ulce´re´es
des deux loges amygdaliennes, pre´dominant a` gauche
(fig. 1), trois ulce´rations infracentime´triques sensibles a` la
palpation situe´es a` la jonction palais dur – palais mou de
part et d’autre de la ligne me´diane (fig. 2). Enfin, le patient
pre´sentait une discre`te inflammation du frein de le`vre
supe´rieure.
La palpation cervicale mettait en e´vidence une ade´nopathie
spinale gauche indolore.

Risk factors for cancers of the oral cavity, pharynx (cavity
excluded) and larynx
Objective > To review the risk factors for squamous cell carcinoma
of the oral cavity, pharynx, and larynx.
Methods > Review of the literature using the Medline digital
database (1980–2007). Previously published studies or studies not
found in the database were included if relevant. Four types of
studies were selected: (1) epidemiological, (2) toxicologic, (3)
clinical, and (4) fundamental research. Publications concerning
cancer of the nasopharynx were excluded. This work is based upon
the ANAES guide for analysis of the literature and rating of
guidelines, published in January 2000.
Results > The principal risk factors are tobacco and alcohol. Other
risk factors, particularly infectious (viral) or environmental (nutritional
and occupational), are also involved. From this analysis we
conclude that: (1) most clinical and fundamental publications
concern smoking and alcohol use; (2) studies of other risk factors are
relatively old, especially those concerning nutritional and occupational
factors; (3) most publications have a low level of scientific

Objectif > Faire le point sur les facteurs de risque des carcinomes
épidermoïdes de la cavité buccale, du pharynx et du larynx.
Méthodes > Revue de la littérature à partir de la base de données
informatisée Medline (1980–2007). Des études antérieures citées
dans les articles retenus, ou ne faisant pas partie de la base de
données, ont été incluses en fonction de leur pertinence. Quatre type
d’études ont été sélectionnés : (1) études épidémiologiques ; (2)
études toxicologiques ; (3) études cliniques ; (4) recherche fondamentale.
Ont été exclues de ce travail toutes les publications relatives
au cancer du rhinopharynx. Notre travail s’est appuyé sur le guide
d’analyse de la littérature et gradation des recommandations, publiée
par l’Anaes en janvier 2000.
Résultats > Les facteurs de risques principaux sont le tabac et l’alcool.
D’autres facteurs en particulier infectieux (virus) ou environnementaux
(nutritionnels et professionnels) sont également impliqués. Il
ressort de notre analyse que : (1) l’essentiel des publications cliniques
et fondamentales portent sur le tabac et l’alcool ; (2) pour les autres

INTRODUCTION: The European Board of Oro-Maxillo-Facial
Surgery (EBOMFS) in common with other European Boards
of recognized UEMS-Specialties is conducting RQ (Recognition
of Qualification) – Assessments. The European Board
exists to enhance the standards of training and practice
through different means. Among those is the EBOMFS –
Assessment, which will give the title of “Fellow of the
EBOMFS“. The Assessment is voluntary and does not affect
free movement of doctors in Europe.
ELIGIBILITY: Candidates for the R.Q. – Assessment must
satisfy one of the three following terms.
— Recognized specialists in oral and maxillofacial surgery
in one of the countries of the E.U. where the specialty is
based on a medical and dental degree.
— Recognized specialists in oral and maxillofacial surgery
in one of the countries of the E.U. where the specialty is
based on a medical degree.
— Recognized specialists in oral and maxillofacial surgery
in one of the countries of the E.U. where the specialty is
based on a dental degree, provided this specialist also holds
a medical degree.
The candidate for the R.Q. – Assessment shall have been
a recognized specialist in oral and maxillofacial surgery for
at least 3 years. It shall be demonstrated that he or she has
effectively practised oral and maxillofacial surgery during
this period. It shall be proved that he or she has shown continuing
scientific and/or continuing medical training interest
in the specialty.
STRUCTURE: The assessment will be in three parts.
1. Curriculum vitae and logbook in English.
2. Multiple Choise Exam (MCQ) in simple English.
3. Oral interview of the candidate.
The Oral Examination will be carried out using one out of
the following four languages: English, French, German,
Spanish. Exceptionally, if the candidate cannot cope with
any of these four languages and do not agree with the
offered suggestion should contact the Secretary General
for further proposals in order to find appropriate examiners
for Oral Assessment. The oral interview will last one hour
and consists of two parts:
A. Questions convering the whole scope of the specialty.
B. Case report. Each candidate must be ready to present
three well documented personal cases: (using slides, photographs,
medical imaging, casts, histology, laboratory findings
etc…).
Candidates who do not pass the first part will not be permitted
to enter the second part. Each part of the assessment
will be conducted by a panel of three assessors
selected by the board. The chairman of each jury will be
from the same country as the candidate. The decision of the
assessors will be final.
The fee for the assessment will be 400 € payable with the
application for the assessment. A copy of the bank draft
or a cheque must be enclosed. The fee cannot be refunded.
The deadline for Application for the 2006 Assessment will
be the 1st of February 2006. The assessment will take place
just before the XVIII EACMFS – Congress in Barcelona,
Spain, from Sept. 12th to Sept. 16th, 2006. Specialists
wishing to enter this assessment or wishing to obtain
further instructions and application forms should contact
the Secretary General of EBOMFS of their national representatives.
